General Job Statement:
Responsible for outreach to adults and families residing in North Minneapolis to

facilities and schools. Presentation can be one-one or group level interventions.
Identify adults and families in need of health and social services and connect them

Qualifications:
- Must have high school diploma or G.E.D.
- Community Health Worker certificate from qualified school

- Good knowledge of interviewing and recording techniques; of the surrounding community; program policies; and some knowledge of health care systems.
- Ability to execute outreach plans, collect data and prepare reports.

- Must have a valid driver’s license and transportation

- Plan, prepare and implement projects and procedures for effective community outreach and enrichment.

DEFINITION:
Under direct supervision of the Community Health Supervisor, this position will conduct outreach activities to link individuals with PCP and increase understanding of preventative and maintenance health care and dental services. May involve helping individuals arrange for services and navigate through complex county, clinic, and managed care organizations.
ESSENTIAL FUNCTIONS:
- Provides Program Service Coordination. Documents all necessary information supporting eligibility for services provided. Schedules program-related meetings, presentations, and outreach activities. Maintains contact with affected Human Service internal divisions, external agencies, community groups, support groups, and other related professionals. Supervises program-related activities of support staff and students.
- Performs Intakes and Referrals. Performs client intakes for new clients. Screens new health clients seeking information regarding their health needs. Explains available services and eligibility factors to clients. Determines client eligibility through an analysis of the information collected within agency and work unit eligibility criteria. Opens and services case for clients/families meeting eligibility criteria. Offers information regarding other service options to clients/families not meeting agency and work unit criteria.
- Performs Record Keeping and Reporting. Performs necessary record keeping and reporting. Develops and maintains case files, completes required agency forms, and submits reports to other providers and state departments. Ensures all relevant documents are maintained in files and current including letters, medical reports, test results, case plans, and case narratives.
- Responds to Emergencies. As directed by supervisor, participates in disaster support, communicable diseases, outbreaks, and investigations in whatever role designated. Works with communities to provide support in areas of need. Assists with planning, consulting, investigating, and providing technical assistance.
- Performs Public Health Functions. Uses public health theories and processes to practice. Assesses health service needs of individuals and groups. Formulates plan/intervention based on data collection and assessment. Participates in preventative health care and health promotions.
- Provides Preventive Health Care and Case Management Services. Provides education to families and the community regarding prevention of health risks. Responds to reports from the state by contacting families. Accompanies state inspector on home assessments and provides proper health information. Follows up with families who have an individual with a health problem. Assists families in implementing measures to decrease risk of further problems.
- Interacts Positively with Internal and External Customers. Communicates and interacts effectively and respectfully with employees, supervisors, individuals from other organizations, and customers.
- Extract and combine data and information from multiple sources. Independently extract data from Public Health programs and other data sources in order to identify potential clients and families. Combine data sets from different systems to identify subgroups, individuals, or populations that meet specific criteria. Analyze data for trends, information, or insight into the health care practices of individuals or populations. Utilize data to guide outreach activities. Update client data as new information becomes available.
- Conduct data analysis and interpretation. Uses data from a variety of sources for analysis of efficacy of services provided, community needs assessment. Manipulates data to develop and generate ad hoc reports to address specific health promotion, outreach, or outcome reporting needs. Incorporates utilization data from UCare into program objectives. Utilizes MS Office products to manipulate data for statistical analysis using appropriate statistical methods. Interprets data and monitors data integrity as part of data analysis.
- Facilitate exchange of data. Downloads data from PH systems and formats according to predetermined standards for exchange with UCare, providers, or state. SUMMARY
The Community Health Worker(s) are part of FCHS’ Stroke Prevention Project, funded by a grant of the Minnesota Department of Health, Eliminating Health Disparities Initiative—currently through 2009. This Project seeks to increase understanding of cardiovascular disease and diabetes in the African American and African born adult communities on the Northside of Minneapolis and surrounding Northwest suburbs. Under the direction and supervision of the Stroke Prevention Coordinator (RN), Community Health Workers (CHWs) provide a critical link to adults who are not connected to a health provider, who may not be aware that they have high blood pressure or other risk factors for stroke and diabetes, and/or who are not getting treatment for a diagnosed and/or under diagnosed condition. CHWs provide initial awareness and education with their peers, neighbors, family, and friends; assist in community-based classes/education and conduct prevention awareness screenings.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Complete the initial peer education training and ongoing training sessions and demonstrate competency.
- Identify and help secure potential community locations for group screening and education events.
- Identify and help forge relationships with local businesses to participate in stroke awareness activities/outreach.
- Perform outreach and education activities at local barbershops
- Assist in planning and implementation of community-based classes/education sessions, presentations, health fairs, etc.
- Assist in community-based stroke risk screenings.
- Facilitate access to primary health care, health insurance, and related community resources for community participants.
- Provide feedback on the education and screening processes and materials.
- Complete data collection forms and submit all required data on time, according to agency requirements.
- Attend and participate actively in stroke prevention team meetings or other agency meetings, as required.
- Perform other related duties as assigned.
REQUIRED SKILLS, ABILITIES AND EXPERIENCE:
- Graduation from an accredited Community Health Worker program.
- Experience as a CHW in a community health setting,
- Demonstrated cultural competency with African American/African communities.
- Residence in the geographic communities to be served (Northside of Minneapolis or Northwest suburbs) is preferred.
- Mature, older adult is a bona fide requirement.
- Motivated to improve the health of the community.
