General Job Statement:
Responsible for outreach to adults and families residing in North Minneapolis to
increase awareness and the need for health prevention, medical and dental check-
ups, social and economic issues impacting their families.  Outreach activities will be
conducted in neighborhoods, door-door, agencies, churches, park and recreation
facilities and schools.  Presentation can be one-one or group level interventions. 
Identify adults and families in need of health and social services and connect them
with NPHWC clinic or human services staff.  Perform assignments related to the
service goals and objectives of the program as contracted.

Qualifications:

  • Must have high school diploma or G.E.D.
  • Community Health Worker certificate from qualified school
  • Must possess demonstrated ability to relate to individuals and families of varied ethnic, cultural backgrounds, ages and economic circumstances.
  • Good knowledge of interviewing and recording techniques; of the surrounding community; program policies; and some knowledge of health care systems.
  • Ability to execute outreach plans, collect data and prepare reports.
  • Excellent oral and written communication skills
  • Must have a valid driver’s license and transportation
  • Must be proficient in MS Word, MS Excel and client database software

SUMMARY
The Community Health Worker(s) are part of FCHS’ Stroke Prevention Project, funded by a grant of the Minnesota Department of Health, Eliminating Health Disparities Initiative—currently through 2009.  This Project seeks to increase understanding of cardiovascular disease and diabetes in the African American and African born adult communities on the Northside of Minneapolis and surrounding Northwest suburbs.  Under the direction and supervision of the Stroke Prevention Coordinator (RN), Community Health Workers (CHWs) provide a critical link to adults who are not connected to a health provider, who may not be aware that they have high blood pressure or other risk factors for stroke and diabetes, and/or who are not getting treatment for a diagnosed and/or under diagnosed condition. CHWs provide initial awareness and education with their peers, neighbors, family, and friends; assist in community-based classes/education and conduct prevention awareness screenings.
