Purchase CHW Curriculum

Purchase the Minnesota Community Health Worker Curriculum:

The cost is $400 which covers the cost of the CHW teaching manual with all the tools and PPs on a DVD.
In exchange for using the Community Health Worker curriculum, institution and organization will need to sign a copyright term of agreement which cover the following principles:

In Minnesota you must attach documentation verifying accreditation of the college or university. In Minnesota we will not release the curriculum to non-accredited training programs. Other states are not obligated to verifying this information.
The copyright of this work is owned by the MN CHW Alliance and the Alliance welcomes use of the curriculum within the constraints of the Copyright Act.
You may use, copy, reproduce, distribute (within your institution) and/or create derivative works of this Curriculum only for non-commercial and academic purposes, subject to the restrictions of the copyright license.
Maintain use of the MN Community Health Worker logo on all training materials.
